Dear Waka Kotahi,

Pursuant to the Official Information Act 1982, I am requesting information relating to the fatal road incident involving Mr. Bryan Marris at the intersection of Hewletts Road and Tasman Quay (State Highway 2), Mount Maunganui, on 28 April 2023.

Based on information reported in the media, it is understood that Waka Kotahi undertook assessments or actions following this incident. Accordingly, I respectfully request the release of any documents or records relating to:

- Any safety assessments or "engineer's reports" carried out by Waka Kotahi or its contractors on the Hewletts Road on-road cycle lane and the Tasman Quay intersection following the incident;
- Documentation detailing proposed or implemented road design changes in response to the incident, including PDF layout plans or concept drawings reflecting the current or intended design state;
- Any internal investigation reports, reviews, or evaluations conducted by Waka Kotahi in the aftermath of the incident.

Should any part of this request require clarification, I am happy to assist. I would prefer to receive the requested information in electronic form where practicable.
