Request made under the Official Information Act 1982 
 
Thank you for your email of 3 July 2025 requesting the following information under the Official 
Information Act 1982 (the Act) pertaining to the fatal road accident involving Mr Bryan Marris: 
 
- Any safety assessments or "engineer's reports" carried out by Waka Kotahi or its contractors 
on the Hewletts Road on-road cycle lane and the Tasman Quay intersection fol owing the 
incident; 
- Documentation detailing proposed or implemented road design changes in response to the 
incident, including PDF layout plans or concept drawings reflecting the current or intended 
design state; 
- Any internal investigation reports, reviews, or evaluations conducted by Waka Kotahi in the 
aftermath of the incident. 

 
The documents outlined in the Document Schedule appended to the bottom of this letter fall within the 
scope of your request and are enclosed. Graham O’Connel , our Manager of System Safety and 
Performance, would be happy to discuss these documents further or walk you through them as

Certain information within attachments 1, 2, 3 and 5 has been withheld under section 9(2)(a) of the Act 
to protect the privacy of natural persons, including that of deceased natural persons.  
 
Certain information in attachment 3 has also been withheld under section 9(2)(g)(i) of the Act to 
maintain public affairs by protecting the free and frank expression of opinions by Government 
employees (including the Minister). 
 
With respect to the information that has been withheld, I do not consider there are any other factors 
which would render it desirable, in the public interest, to make the information available. 
 
Under section 28 of the Act, you have the right to ask the Ombudsman to review my decision to
