Workplace Death of Murray Ian Christensen at 590 Koheroa Road,
Mercer on 11/11/11

Please note on Officer's Evidential Statement Continuation Form (and we have had direct contact with her also), the following, "ASN1, Senior Sergeant Ian Brenchley, ASH16, Constable Colin Pegg and AHS23, Constable Steven Latham arrived on scene. I briefed Senior Sergeant Ian Brenchely, who then took over as Incident Controller. We conducted a scene examination to establish what had occurred"

Please provide to the writer the following details and information in accordance with the Official Information Act 1982 as set out in Commissioner of Police v Ombudsman (1988).

Details of the physical exhibits including those noted in Cherie Elisabeth McLean's Officer's Evidential Statement which states that a "POL268 was completed and the deceased's property was seized. The property included: gumboots, socks, t-shirt, metallic bracelet, wrist watch and a mobile phone. All property was secured in the Pukekohe Exhibits Store".

An explanation of why the above Evidential Statement by Cherie Elisabeth McLean's conflicts with that of the Initial Report by Coroner which states, "All property (except t-shirt) was seized and taken to Pukekohe Exhibits Store". Please provide a copy of the Police Document POL268 to establish if the t-shirt was secured in the Pukekohe Exhibits Store or not. Which of these two Official Police Documents is incorrect and therefore a false declaration?

As the Incident Controller, we request a full explanation of where Murray's green jumper went. It was clearly alongside Murray's body with the t-shirt in the Official Police photos. Please provide full details of where the green jumper was secured and if it wasn't, why not? Why was it not given to Cherie Elisabeth McLean to secure with the t-shirt in the Pukekohe Exhibits store?

The Initial Report for Coroner states Suspicious Death: No. Please
then explain and give a full explanation as to why you, as the Attending Supervisor/CIB Investigator (as noted in the Initial Report for Coroner), attended the scene and took over from Cherie Elisabeth McLean, as Incident Controller if the Death was not suspicious.

Please explain why the scene was not frozen? Or if it was, why then was the green jumper not secured in the Pukekohe Exhibits Store along with the T-shirt seen in Official Police Photos.

Did you complete an Evidential Statement? Yes Or No? (If yes, please provide a copy of your Evidential Statement. If No, please give a reason as to why you did not complete an Official Evidential Statement relating to the workplace death of Murray Ian Christensen at 590 Koheroa Road, Mercer on the 11/11/11)

Did you complete any Police job sheet/notes in connection to the above case? Yes or No? (If yes, please prove a copy of your job sheets/notes. If No, please give a reason as to why you did not complete a job sheet/notes in connection to the above case)

We require a full explanation as to why the Initial Report for
Coroner has under Fatal Death: NO. Please provide all information
and copies of documents which support that this was NOT a fatal
death at 590 Koheroa Road, Mercer, or if this was a false
statement, please give details and a clear explanation as to the
reasons for this false declaration on an Official Document for the
Coroner.
