Dear Fire and Emergency New Zealand,
In Feb 2018 a major fire occurred on the Chathams Island (the Makara Fire). You have advised that the specific costs associated with the response to this fire totalled some $1,400,000. You also advised that while your own documented policy indicates that this "may" be held, no formal and independent Operational Review of this fire was undertaken. This is very surprising given the very significant cost of the Makara Fire and the significant logistical issues with a response to the Chathams. It is also noted that the Makara Fire meets the following FENZ criteria of an Operational Review:
High level of incident complexity existed for example:
(a) fire suppression costs (Makara Fire = $1.4 Million)
(b) property damage
(c) area burnt (Makara Fire = 3000 Hectares);
and;
When there has been significant environmental damage;
and;
When it is considered that the outcome will benefit others through the sharing of
lessons learnt.
Under the provisions of the Official Information Act I request:
1. The documentation that FENZ has (eg correspondence, memorandum, meeting minutes, notes, etc within the FENZ Management Team that's sets out the basis for an Independent Operational Review of the Makara Fire. This documentation should include the supporting arguments, recommendations and/or decision not to undertake such a formal and independent review.
2. Copies of the Incident Action Reports (IAPs) for the duration of the fire; and
3. A map overlay of the final extent of the fire (if not included in the IAPs).

Dear FENZ,
The attachment (which contains the requested IAPs) is way too large for a stable PDF and can not be downloaded from the FYI site because of its size. Any attempt to do so returns an error.
Can you please break up this file into 3 or 4 smaller sized documents to enable them to be downloaded.
