Kupe Statue Incident Wellington Waterfront

Dear WorkSafe New Zealand,

I refer my Official information request dated June 23 2023, and your subsequent responses. I asked;

"I am a NZ citizen seeking information on incidents on the waterfront that have been reported and/or notified to Worksafe by Wellington City Council, or its subsidiaries in the year 2021 in respect of the Wellington Waterfront.
This should include all incidents where Worksafe have been notified of people falling into voids or gaps in the areas and wharves around the waterfront.
Of particular interest is around or on the Taranaki Street Wharf.
• This should include:
1. Date and time of the incident
2. Date and time it was reported
3. Action taken by Worksafe in respect of the incident
4. Any reports provided to Worksafe in respect of the incident
5. Any reports produced by worksafe in respect of the incident Your request has been considered under the Official Information Act 1982 (OIA)":

You responded only with information about a notification about my son's death on the waterfront in July 2021. And the reasons why you refused to investigate.

I now refer you to the following article from the Herald reporting from my Son's coronial inquest last week. The report talks of evidence given about an incident where a member of the public fell into a void in the wharves on the waterfront at the Kupe Statue and was injured quite badly due to a contractor having forgotten to turn lights back on the previous week. The report states:
"Shaw asked if the Kupe incident had gained attention because WorkSafe was notified, “but these other very serious, fatal tragic incidents, which on one categorisation didn’t trigger a mandatory report to WorkSafe, simply got left.”

It is concerning to me that Worksafe appears not to have met it's statutory obligations under the Official Information Act 1982 in respect of my request of June 23, 2023.

Can you please answer the questions from my original request as restated above in respect of the Kupe Statue incident on 10 July 2021. And can you please advise why my original request did not see this information provided as required by law?
