Could you please provide the following:
Number of notifiable events and injuries made to work safe for the past 5 years by the PCBU Health New Zealand, including any previous notifications made by DHB's during this period.
Please provide brief details/investigation files for each incident, and specify whether the incident/injury prompted an investigation from work safe.
If the request is to be refused for substantial collation and research, please just provide the number of notifiable events and incidents notified by health New Zealand in the past 3 years, and the incidents/injurie details/investigation files from Health New Zealand Southern.
