Thank you for your Official Information Act request received on 12 August 2024 regarding an
incident at the Kupe Statue on the Wellington waterfront on 10 July 2021.
You requested:
Can you please answer the questions from my original request as restated above in
respect of the Kupe Statue incident on 10 July 2021. And can you please advise why my
original request did not see this information provided as required by law?
Your questions from your original OIA request of 23 June 2023 (reference 230611) were:
1. Date and time of the incident
2. Date and time it was reported
3. Action taken by Worksafe in respect of the incident
4. Any reports provided to Worksafe in respect of the incident
5. Any reports produced by worksafe in respect of the incident
Firstly, please accept my deepest condolences for the loss of your son, Sandy.
It is regrettable that the details regarding the notification of the Kupe Statute incident were not
provided in response to your original request. This information was not identified at the time of
your earlier request, and I acknowledge that this omission will likely have been frustrating for you.
WorkSafe receives a large number of notifications and this information is recorded and managed
for the purpose of enabling WorkSafe to undertake its regulatory functions. While our records are
appropriate for our regulatory purposes, how our data and information is recorded does not
necessarily enable it to be easily retrieved for other purposes. Effective searches of our data can
be affected by several parameters including technical limitations on search fields, and
inconsistency between details recorded and search terms used. Searches will often require a
manual exercise, which presents potential for human error and oversight.

Injury/Illness sustained by someone
tripped over edge definition at edge of a lighting pit. fell into pit -
approx 1m below and impacted the metal grate at the bottom of the
pit impacting left eye and forehead above
Wellington Free Ambulance were on site and treated patient and
transported to hospital
