Dear Accident Compensation Corporation,

I welcome the provision of the following official information in relation to Dr Afraz Adam, who disclosed to me as a treated patient that he had previously worked for the Accident Compensation Corporation.

Requested information:

1. The commencement date and or dates, if there was more than one period, that Dr Afraz Adam, commenced employment/contracting, and or started being remunerated for services provided to ACC, and the date and or dates, if there was more than one period, in which Dt Afraz Adam ceased to provide services for remuneration to the Corporation.

2. In relation to the Corporation's answer to OIA request 1. above, please confirm in writing the nature of the employment/contracting relationship that was in place between Dr Adam and ACC, for each of the periods to be specified from OIA request 1. above....ie. was Dr Adam an employee, or was he a contractor, or remunerated on the basis of another legal contract please specify the nature of the contract.

3. In relation to the Corporation's answer to OIA request 1. above, please confirm in writing the position/s, role/s/job description/s and job/s title that Dr Adam held for each period specified in answer to the Corporations response to OIA request 1. above.

I confirm that under s16(2) of the Official Information Act, my preferred way of receiving a response is by email to the address from which ACC received the original request, not by post.

information:
