It would be appreciated if you could provide the Official Information sought in my request today.

Background 1:

On Oct 17 2012 Stuff through the Timaru Herald news website wrote a piece about Mr William "Bill" Taine which can be fund at this web address: http://www.stuff.co.nz/timaru-herald/new....

The article makes it known that prior to retiring from his orthopaedic work, and his role of the chief medical officer for the South Canterbury District Health Board, Mr Willam Taine held a part time role with ACC.

OAI Request 1:

Please confirm the date, month and year which with Mr William Taine first commenced employed and or contracted work with ACC.

OIA Request 2:

Please confirm whether Mr William Taine is still employed by ACC and or whether Mr William Taine still provides contracted services to ACC.

OIA Request 3:

Please advise the date, month and year which with Mr William Taine, should he no longer provide services for the ACC in any manner, have ceased his employment and or contract work with the Corporation.

OIA Request 4:

Please provide confirmation of all job positions/roles and or contracted job positions/roles that Mr Willam Taine has been employed and or contracted in whilst providing services to and on behalf of ACC, provided in successive order that he first held each position/role through to either his last position/role or todays current position should Mr William Taine still provide services for renumeration to ACC currently.

Background 2:

It is noted through the Medical Council of New Zealand's website: https://www.mcnz.org.nz/support-for-doct..., that Mr William Hedley Taine's APC (Annual Practising Certificate) expired on the 10 Dec 2015.

OIA Request 5:

In the event that Mr William Taine has ceased employment with and or contracted work for the ACC, please confirm the date, month and year when Mr William Taine first gave formal notice to the Corporation on his pending termination of employment and or contracted service to and or for ACC.

OIA Request 6:

In the event Mr William Taine has ceased employment with and or contracted work for the ACC, please confirm whether Mr William Taine provided the reason for stoping as his "retirement".

OIA Request 7:

Please provide confirmation in a table detailing on an annual basis the number of times Mr William Taine, was employed and or contracted to provided specialist medical opinion on claimant's request for cover and or entitlements for injuries that were presented to the Corporation for the purpose of seeking cover and entitlements.

Please supply this information for the annual periods commencing 1 July 2012, through until present, and or the year that Mr Taine may have ceased his employment and or contracted services with the ACC.

OIA Request 8:

Please provide confirmation in a table detailing on an annual basis the amount in NZ$ paid to Mr William Taine for his employment and or service for which he was employed and or contracted to ACC to provide specialist medical opinion on claimant's request for cover and or entitlements for injuries that were presented to the Corporation for the purpose of seeking cover and entitlements.

Please supply this information on an annual basis for the annual periods commencing 1 July 2012, through until present, and or the year that Mr Taine may have ceased his employment and or contracted services with the ACC.

I have reread my initial requests and noted that OIA Request 9 was incomplete.

It would be therefore appreciated if you could also provide the following information which was meant to be requested on 8 February 2018;

If Mr William Taine provided services through employment and or contracted work to the Corporation, past 10 December 2015, please confirm in a table detailing on an annual basis the number of times Mr William Taine, was employed and or contracted to provided specialist medical opinion on claimant's request for cover and or entitlements for injuries that were presented to the Corporation for the purpose of seeking cover and entitlements.

I appreciate that todays request will be seen as a new request and am happy to receive the information in full including the remaining 8 OIA Requests on or before the 13 March 2018.
