It would be appreciated if you could provide to me the following official information;

1. The annual remuneration in bands of $25,000 that Dr Vasudeva Shivaraya Pai received from ACC for the ACC Financial Years 1 July 2013 to 30 June 2014, and from 1 July 2014 through until 30 June 2015 in terms of;

a) contracted ACC work that involves ACC clients being referred to Vasudeva Shivaraya Pai for any and all medical assessment,

b) representing ACC at either ACC review and or ACC District Court appeals,

c) any and all other (please specify) contracted work for ACC.

2) The annual remuneration in bands of $25,000 that BONEFIX LIMITED for which Dr Vasudeva Shivaraya Pai is the sole named Director received from ACC for the ACC Financial Years 1 July 2013 to 30 June 2014, and from 1 July 2014 through until 30 June 2015 in terms of;

a) contracted ACC work that involves ACC clients being referred to BONEFIX LIMITED for any and all medical assessment,

b) representing ACC at either ACC review and or ACC District Court appeals,

c) any and all other (please specify) contracted work for ACC.

3) Confirmation of the legal name of any and all other legal entities such as companies, partnerships that Mr V S Pai may have used to contract services to ACC, and for which ACC remunerated such legal entities for, at the same time providing the same information as requested in 1) and 2) a), b), and c) for any such legal entities for the Financial Years 1 July 2013 to 30 June 2014, and from 1 July 2014 through until 30 June 2015.

Please specify the scope of all assessments and contracted work that Dr Vasudeva Shivaraya Pai, BONEFIX LIMITED, or any such entity named in response to question 3 above, had contracted arrangements with ACC, either in place between the period 30 July 2014 and 30 June 2015.

Dear Government Services,

Thank you for your communication received 23 March 2017, advising that Government Services require in total 51 working days to respond to my OIA request of 23 February 2017.

In response to your section 15 (A) notice of extension advising that Government Services require a further 30 working days through to the 9 May 2017 to respond to my OIA Request, I respectfully request that ACC Government Services review the March 23 section 15 (A) extension under urgency pursuant to section 12 (3) of the Official Information Act 1982 and in doing so make the requested OIA information available to me within 28 working days from the date my original request was lodged on the 23 February 2017, being by the end of business close on the 3rd April 2017.

In satisfying the requirements of section 12 (3) of the Official Information Act 1982, when requesting for urgency to be applied for this specific information to be provided on 3 April 2017, I provide my following reasons;

1. I refer you to the following similar OIA request in relation to another ACC Medical Assessor who has also been involved in my assessment on behalf of ACC, as has Mr Pai, that being Mr Gordon Howie. My request regarding Mr Howie can be found at:

Government services in the case of My OIA request re Mr Howie also requested a section 15 (A) time extension, resulting in my OIA regarding Mr Howie lodged through FYI on the 22 May 2016, being responded to by ACC Government Services within 28 working days, and received through my personal email address on 30 June 2016.

2. I have requested this information because Mr V Pai, has played a role in my case management, and I am requiring this information for the purposes of pending review and appeal submissions falling due in early April 2017.

3. Given that the OIA information that I have requested previously for Mr Gordon Howie on 22 May 2016, was remarkably similar and it was reasonably provided to me within 28 working days last year, I respectfully request that ACC provide the same level of timeliness and service with my current OIA request re Mr V. Pai, and return the information that I have request under urgency today on or before the end of business close on the 3 April 2017, based on my reasons provided for the consideration and granting of urgency in these matters.

I am more than happy to take receipt of the Official Information in this instance by way of Government Services emailing the OIA information requested to my personal email addresss being [email address].
