Request for confirmation of current and or prior employment dates and roles within ACC

I am writing to respectfully request the following official information held by ACC with respect to the previous and or potentially current ACC employee/contractor [email address].
I have reason to believe that [email address], has previously held the role of Acting Branch Manager for the Henderson ACC Branch. I have placed the persons name in the ACC email format so that it is not displayed publicly through this website and am happy to have the OIA information that I am seeking returned to my private email address being [email address].
It would be appreciated if you could provide me with the following official information;
1. written confirmation as to when [email address], first commenced employment/contracting with the Accident Compensation Corporation.
2. written confirmation of all job roles that [email address], held whilst employed and or contracted to ACC. Please ensure that they are listed in chronological order and the appointment dates and tenure (eg March March 2004-May 2005) of each role are clearly specified.
3. written confirmation as to when [email address] last held employment and or contracting roles with ACC.
