Dear Health New Zealand,
I'd like to please request my deseased father's (Brian Close) full medical history please. As there is only myself, my sister and my 16 year old son that share his DNA. There is a history of heart disease and cancer throughout the wider family.

National Privacy Office. In order to get this to the right area for
response, can you advise which hospital(s) your father received healthcare
from please? To note, Te Whatu Ora does not have access to GP or private
practice records.
