Dear Waitemata District Health Board,

Our Son Kahu James Harawira passed away on the 14th June 2017. He was a patient of the Mental Health Services from 21 April 2008.

Under the official informations Act - I wish to request (principle of availability)
two full and complete files being:

1) all his mental health records
2) all his medical health records

We request this information be released to me in the "way I prefer" which is in two seperate files 1) his health records and 2) his mental health records
Both of these service records are a priority. I also prefer these files be sent to me in two forms 1) a hard copy and 2) an electronic copy.

Those files to contain all consultations, medical reviews, medications prescribed, reports, memos, letters, note, emails and draft documents, and any other record held on file from your organisation regarding Kahu James.

We are a New Zealand Citizen of Ngapuhi descent and understand that a decision on a request for information under the Offical Informations Act 1982 should be made within 20 working days of receiving that request.

Dear Mr and Mrs Harawira

On behalf of Waitemata DHB, I extend our sincere condolences for the loss of Kahu.

So we can release Kahu’s records to you, could you please provide evidence that you are the executor or administrator of Kahu’s estate in the form of a copy of his will or sealed orders of probate/administration.

If Kahu did not leave a will and you have not applied for orders to administer his estate, please confirm that you are responsible for winding up Kahu’s affairs.

Once we have received this from you, we will be able to release his records.
