Dear Ayesha Verrall,
I would like full disclosure from all hospitals on my father's medical history. Auckland hospital. Most DEFINATELY would like ALL notes/records of when he was admitted to Taupo hospital in December 2021. Why they discharged him given his history. Also why an inquest has not happened. The law states if someone dies within x amount of days after being discharged their will be an investigation. Bilateral pulmonary is that of a few outcomes. One being adverse reaction to medication. I want answers and I won't be ignored any longer or swept under the carpet like it isn't medical misconduct which he never made headlines of man that died alone when he was taken on a 45 minute drive to taupo from turangi by ambulance. He was discharged in an unformiliar place, had to walk, no phone, not feeling good charged $200 for a motel room that took him close to an hour and a half-2 hours to find , waited for motel staff, paid, showered and died all before he could get in the bed. Why was he not offered transport back, that is their thing that they claim to do. And motel cover. I am the oldest of his 2 daughters, and the mother to his only grandchild, my 16yr old son. We want answers, an inquest. Full transparency. And the lakes district to take accountability. I am a single mother on a benefit. I had to borrow a thousand dollars to bring my father back to Auckland. My house recently flooded and I have to live in further hardship to pay yet more debts to refurnish a home again. Things I lost of my dad's as simple as an old t-shirt or an old paint brush. I'm done with being left in the dark. Please respond as this is my second request. I'm done being made to wait for my father's actual last hours.
