Dear Department of the Prime Minister and Cabinet,

The Lesson\NZ report on the Whakaari/White Island Response (Nov 2020) states on Page 19 (Insights Para e) that "Feedback indicates that the application of CIMS worked well for the event". This statement contradicts other statements in the report (eg confusion re who was the Lead Agency, failings with respect to taskings, issues re roles and responsibilities between agencies, coordination centres and IMTs, etc).

Under the provisions of the Official Information Act I request a copy of the "feedback" (as a communication, debrief notes, record of a meeting or other notes) provided to the authors of the report that supports the statement made in Insights e) on Page 19 that "CIMS worked well" along with the name of the organisation(s) who provided the feedback.
