Reporting deaths following vaccination with Cominarty

Dear Ministry of Justice,

If a person dies after receiving the Cominarty vaccine:

(a) Will an inquest be required?

(b) What onus is on the deceased's doctor to request an inquest?

(c) Under what circumstances may that doctor issue a death certificate without an inquest?

(d) How long is the time frame involved in all the above?

Dear Ministry of Justice,

I understand that deaths have to be reported to the coroner (inter alia)

if the person died unexpectedly during surgery or medical treatment or while under anaesthetic, or if they died unexpectedly after surgery or treatment and apparently as a result of it.
