Post Mortem Analysis - Coronial Process & Number of Deaths Investigated

Chris McCashin made this Official Information request to Ministry of Health

This request has an unknown status. We're waiting for Chris McCashin to read recent responses and update the status.

From: Chris McCashin

Dear Ministry of Health,

As at todays date the Medsafe reports confirm that there have been a total of 171 deaths reported to CARM after the administration of the Comirnaty vaccine. To any lay person this is a massive red flag and alarm bells are ringing. However the same alarm bells do not seem to ring with the Ministry of Health? The narrative is slowly changing in mainstream media with death and injury now more widely reported and it is only a matter of time before "conspiracy theorists" are going to be proven right again with the damn bursting as to the massive cover up this is. Despite this, our public health agencies continue to be marketing & PR executives for big pharma all the while more and more people are dropping dead. I can even refer you to OIA requests I put in alluding to heart attacks will start to occur in children more frequently which is now starting to happen. This was something I was hoping I wouldn't be proven correct along with the other people working tirelessly to prove this.
However sadly this will be the case and pretty soon I should have enough evidence to prove it. Therefore are you able to provide the following. Of the summary of reported deaths are you able to provide the following.

Please provide the following

- Number of the 171 deaths referred to the Coroner - if these weren't referred to the Coroner what is the process? I have been referred to births, deaths and marriages as a few sudden deaths where I requested the Coroner files available for public they stated they weren't referred these cases? If the case goes to births, deaths and marriages what process do they go through for these deaths? Have DIA been involved in providing settlements to these people as it doesn't seem to appear through MOH or ACC? A yes or no answer will suffice.
- Number of the 171 deaths where the Coroner decided if a post mortem was needed
- Number of the 171 deaths where the family has requested a post mortem - what is the process for a family member to obtain a post mortem
- All of the Post Mortem analysis completed by the pathologists, tissue samples, stains, any and all process documents associated with completing the Post Mortem analysis
- Number of deaths Coroner has said an enquiry is needed versus not needed - eg 171 inquiries are needed versus 0 not needed - one would assume all of these sudden deaths would be needed?
- Total number of Post Mortem reports completed associated with the 171 deaths
- There are 23 deaths "that could not be assessed due to insufficient" information. I can tell the Ministry of Health all of the tests that need to be performed that can determine causality from the vaccine - subject to this response is the Ministry of Health interested in finding out why these 23 people died if I can provide a comprehensive list of medical tests to perform? Surely this is in the public interest as 7 years olds, and 12 year olds are now dropping dead?
- Per Section 14 of the Coroners Act all of these deaths appear to be the result of a "medical procedure" - based on Medsafes own data at a minimum 34 deaths could be linked to this medical procedure however there could be in excess of 3,000 due to this procedure. What is the "minimum" standard associated with unexpected deaths by a medical procedure for the Coroner to make a recommendation - 1 death, 2 deaths, 3 deaths? 171 deaths in strange circumstances?

Yours faithfully,
Chris

Dear OIA Requests,

A follow up on this - given this treatment is a new novel technology and there are so many dead within 21 days I would have thought all of these deaths would HAVE to be referred to the Coroner?

It is unbelievable if they haven’t. One case of Covid-19 shuts down the country, 177 deaths including children and nobody bats an eyelid at MOH. I will try to rephrase

Of the 177 deaths
How many were of these occurred where the

- Report was not validated due to address not being verified, no NHI number or failed follow up with reporter per the flow diagram provided under another OIA request
- GP of the deceased was attempted to be contacted (2 x minimum) - not contacted so therefore went to the ISMB for input and the case was closed per the flow diagram
- Signed off by a GP or Health Practitioner and closed or classed as “unexpected or unexplained”

Can you also clarify the following

- What is the criteria for a person to be classed as vaccinated? For example there are in excess of 3,000 deaths within the ‘Observed’ category during the monitoring period. If for example somebody dies within 7 days are these immediately discounted
as being from the vaccine due to the person not being defined as vaccinated or are all reports of death after vaccine considered if someone reports?

- And also - how are the observed deaths picked up?

Please provide associated process documents eg

- Person vaccinated
- MOH follows up to see if person is feeling okay within monitoring period
- MOH doesn’t hear back then what happens
- MOH contact GP / family?
- MOH finds out someone has died?
- GP / friends and family did not know the person was vaccinated in last 21 days so no report is filed and it’s assumed natural causes
- Do MoH notify friends / next of kin that the person was vaccinated and they report it to GP / Coroner
- Or do MOH not disclose and the death is added to observed in monitoring period

Please provide all steps taken and what is done to pick up observed deaths and the process following.

Yours sincerely,

Chris McCashin

Tçnâ koe Chris,

Thank you for your OIA request, which was partially transferred to the
Department of Internal Affairs on 11 November 2022, in respect of the
below information

“If the case goes to births, deaths and marriages what process do they go
through for these deaths?

Have DIA been involved in providing settlements to these people as it
doesn't seem to appear through MOH or ACC? A yes or no answer will
suffice.”

Births, Deaths and Marriages (BDM) registers information in accordance
with legislation, specifically the Births, Deaths, and Marriages
Registration Act 1995 (BDMRRA). Should you be interested in the BDMRRA, it
is available to view online here
[1]https://www.legislation.govt.nz/act/publ...

For death records, BDM registers the cause of death as per the information
provided on the Notification of Death for Registration. I must stress that
BDM does not determine the cause of death. For death records where the
cause of death field has been registered with “Subject to Coroner’s
Findings”, this will be amended once the case is resolved by the coroner’s
office.

I can advise that the Department is not involved in providing settlements,
as stated above, BDM registers information in accordance with the BDMRRA.

Please note that in cases where the Department’s response provides
information that is identified to be of general public interest, the
response may also be published on the Department of Internal Affairs
website.  If the Department publishes its response to your OIA request,
all personal information, including your name and contact details, will be
removed.

If you are unsatisfied with the Department’s handling of your request,
information about how to make a complaint is available at
[2]www.ombudsman.parliament.nz or Freephone 0800 802 602.

Thank you for your request asking for the following information under the
Official Information Act 1982 (the Act):

1. You state there were no Covid memorandums prior to 7 May 2021.  So
please
provide me the scope of works with the CV-ISMB, how much they were paid by
that
date, deliverables associated with CV-ISMB - what did they do if they
didn't
produce one memo in the first five months of them being paid?
2. Medsafes own data confirms that TTS has occurred from the Pfizer
vaccine -
how can you say this isn't a side effect from the Pfizer vaccine when
there are
over 60 side effects registered on your own data that this is a side
effect?
5. Please provide data as requested on the anaphalaxis - how many cases of
anaphalaxis were not recorded when measuring against the Brighton
Collaboration
6. Were people prior to vaccination warned of all the safety signals?
7. A safety communication was issued - are these safety communications
relayed
to people prior to vaccination? If not, why not?
10. I have read the minutes you don't need to link me through to stuff you
have
sent - what is the average "under reporting" rate of vaccine injuries for
this
medication and previous medications
11. This means nothing - you are studying people after you have injured
them - a
400% increase in Myocarditis and likely numerous delayed deaths through
cardiac
arythmia in children is going to occur going forward in New Zealand and is
already happening overseas
12. Whilst the information is publically available are vaccinators warning
of
Myocarditis so that New Zealanders get informed consent?
13. Please provide appendix one
14. I have read the safety signals - however are people warned of the
safety
signals per informed consent process

This email is to let you know that Te Whatu Ora needs more time to make a
decision on your request.

The Act requires that we advise you of our decision on your request no
later than 20 working days after the day we received your request.
Unfortunately, it will not be possible to meet that time limit and we are

Thank you for your email on 4 November 2022, asking for the following
information under the Official Information Act 1982 (the Act):

"I will review the studies with funding / conflicts and so on

Of the 132 I need to know a sequence of events

Such as

- arrived with chest pains to be checked out at hospital
- had a test and tested positive for Covid-19
- what is their vaccination status
- how many of the 132 cases are recorded in the Medsafe report as from
vaccine? It isn’t clear, for all I know these 132 cases could be 132 of
the cases that are recorded in the list of AEFIs

Despite the spin you are trying to put on things.  Myocarditis is a side
effect from the vaccine.  This is a fact.

Simple question - how many of the 132 were vaccinated?

Everyone at the Ministry of Health should be looking at testimony of
Pfizer executives overseas along with some of the lawsuits that are taking
place."

This email is to let you know that Te Whatu Ora needs more time to make a

The Act requires that we advise you of our decision on your request no

Tçnâ koe Chris,

Thank you for your request under the Official Information Act 1982 (the
Act) to Manatû Hauora (the Ministry of Health) on 16 November regarding
COVID-19 deaths, processes, and documentation. You requested:

“Can you also clarify the following

- What is the criteria for a person to be classed as vaccinated? For
example there are in

excess of 3,000 deaths within the ‘Observed’ category during the
monitoring period. If for

example somebody dies within 7 days are these immediately discounted as
being from

the vaccine due to the person not being defined as vaccinated or are all
reports of death

after vaccine considered if someone reports?

The criteria of observed and expected analysis of COVID-19 deaths is
outlined here:

[1]www.medsafe.govt.nz/COVID-19/vaccine-report-overview.asp.  Please note
there is no assessment of causality in the safety report, as this is a
purely a statistical analysis.

The remainder of your request will be responded by Te Whatu Ora in due
course.

Under section 28(3) of the Act, you have the right to ask the Ombudsman to
review any decisions made under this request. The Ombudsman may be
contacted by email at: [2][email address] or by calling 0800
802 602.

Kia ora Chris,

Thank you for your request under the Official Information Act 1982 (the
Act) on 4 November 2022 for information regarding the COVID-19 Vaccine:

1. You state there were no Covid memorandums prior to 7 May 2021.  So
please provide me the scope of works with the CV-ISMB, how much they were
paid by that date, deliverables associated with CV-ISMB - what did they do
if they didn't produce one memo in the first five months of them being
paid?
2. Medsafes own data confirms that TTS has occurred from the Pfizer
vaccine - how can you say this isn't a side effect from the Pfizer vaccine
when there are over 60 side effects registered on your own data that this
is a side effect?
5. Please provide data as requested on the anaphalaxis - how many cases of
anaphalaxis were not recorded when measuring against the Brighton
Collaboration
6. Were people prior to vaccination warned of all the safety signals?
7. A safety communication was issued - are these safety communications
relayed to people prior to vaccination? If not, why not?
10. I have read the minutes you don't need to link me through to stuff you
have sent - what is the average "under reporting" rate of vaccine injuries
for this medication and previous medications
11. This means nothing - you are studying people after you have injured
them – a 400% increase in Myocarditis and likely numerous delayed deaths
through cardiac arythmia in children is going to occur going forward in
New Zealand and is already happening overseas
12. Whilst the information is publically available are vaccinators warning
of Myocarditis so that New Zealanders get informed consent?
13. Please provide appendix one
14. I have read the safety signals - however are people warned of the
safety signals per informed consent process

Pursuant to section 15(1)(a) of the Act, Te Whatu Ora is writing to advise
you that a decision has been made on your request; however, it will take
us some time to prepare the information for release. We will send you the
information as soon as reasonably practicable.

Under section 28(3) of the Act you have the right to ask the Ombudsman to
review any decisions made under this request. The Ombudsman may be
contacted by email at: [1][email address] or by calling 0800
802 602.
