I was reading through Lessons Learnt for the transfer of the National Emergency Management Agency over to DIA (Hukatai reference YYY27J26TNJA-1667321898-1888) and noted Paragraph 49:

I would also like to request any relevant documentation, which may include but not be limited to briefings, memos, discussion documents, incident reports etc regarding the referenced incident. This includes both the specific inability to access the emergency environment as well as any documentation about inadequencies of the work done by the vendor and remediations taken.
