I am writing to request information on how an individual is able to obtain full copies of their complete and unabridged ACC 45 screen shots with both Display Sub-Case Documents and Display Removed Documents indicators ticked, inclusive of all tabs listed, but not solely limited to those shown below;

Case Map
General
Entitlements
Plan
Managing
Contacts
Documents
Tasks
Injury
Medical
Employment
Treatment
Documents for claim
Document Groups….etc

So that an ACC claimant is able to receive in chronological order the best summary of what has been sent from ACC and what has been received to ACC and from whom.

It would also be appreciated if you could confirm what timeframe would be considered reasonable by Government Services for an individual to expect that the above information would be readied for thier collection, or alternatively be able to be couriered to the individual to whom had requested the information.

At the same time would you please confirm whether the screenshots referred to form part of a persons Party Status Files, or form part of a persons EOS files.

There are also tabs to the right of the Documents screenshot that are obscured from view and I would appreciate receiving a copy of a training template which details the full screen inclusive of all tabs.

Dear Government Services,

I attach a copy of the ACC45 Claim Treatment Screenshot PDF that I referred to in my OIA requests lodged through FYI on;

18/12/15 titled: Requesting and receiving full copies of the following screenshots

and

21/12/15 titled : ACC 45 claim screenshots: How an ACC client can obtain full copies.

It has recently and rightly been pointed out to me that Government Services most probably will not have password access to open the PDF at the website I have referred you to.

Therefore I have attached a copy of the ACC45 Claim Treatment Screenshot PDF (referred to as the PDF acc45screenshot document in my request of 18/12/15) to ensure that there is no unwarranted delay in Government Services responding to my OIA requests on behalf of the Accident Compensation Corporation.

Thank you for your reply.

In response to this request which you have referred to as Request 4, on the last page of your letter under Question IV: Template of Eos screen, you have written and I quote;

Please find an image of a full documents screen from Eos attached, showing all tabs displayed on this screen. For privacy reasons, ACC has blacked out names of identifiable people. The full details though of this screen are provided in accordance with this part of your request.

It is noted that no such image was supplied or attached to your Government Services response received 19/02/16, and therefore I request under urgency that you provide a copy of the Documents screen section to which you refers by no later than the end of business close on 22nd February 2016, to remediate any breach of your obligations under the Official Information Act 1982, that will come into effect at end of business close today.

Under s16(2) of the Official Information Act, my preferred way of
receiving a response is by email to the address from which ACC
