ACC 45 claim screenshots: How an ACC client can obtain full copies.

I refer to the ACC45 Claim Treatment Screenshot PDF at the following website address;

OIA Request 1)
It would be appreciated if you could confirm whether these screen shots form part of the ACC Party level screens or the EOS screens.

The referred ACC45 Claim Treatment Screenshot PDF shows the Documents, Documents for Claims screen and the Contacts screen.

Both screens include the following tabs;

Case Map,
General,
Entitlements,
Plan,
Managing,
Contacts,
Documents, which also show Documents for Claim and also Document Groups
Tasks,
Injury,
Medical,
Employment
With an arrow tab to the right suggesting further tabs.
There are also other screen tabs on the Documents screen which have been cut off when the screen shot was taken.

The screenshots show the chronological order in which information was received to and forwarded from ACC, together with details of who or where the information was received or sent by.

The Documents screen has two additional tabs;

a) Display Sub-Case Documents, and
b) Display Removed Documents.

which can be clicked to provide the person viewing the screen the most accurate flow of information in chronological order.

The Contacts screenshot has the following two tabs which can be clicked;

a) This case, and
b) This case and it's sub cases.

Both sets of screenshots have noted in the bottom right hand corner the number of pages i.e. 1-21 of 21 etc.

OIA Request 2)

It would be appreciated if you would provide me with all information as to how an ACC client can request the timely receipt of copies their full, complete and unabridged copies of of all screenshots for all tabs (i.e. including but not limited to Case Map, General, Entitlements, Plan, Managing, Contacts, Documents, which also show Documents for Claim and also Document Groups, Tasks, Injury, Medical, Employment, and any other screen tab that is associated with these screens but not shown), for each ACC45 Claim, with both the Display Sub-Case Documents and Display Removed Documents tabs selected so that full information is forwarded to the ACC Client.

So to recap OIA Request 2- please advise how an ACC Claimant can receive copies of their full, complete and unabridged screenshots held/stored under all tabs noted above and any further that are not shown on the ACC 45 Screenshot PDF example.

The attachments on that forum are not downloadable without logging in, and I doubt ACC is going to create an account there to respond to your request. You may need to put the files somewhere else they can download them. (FYI can't send attachments.)

I attach a copy of the ACC45 Claim Treatment Screenshot PDF that I referred to in my OIA requests lodged through FYI on;

18/12/15 titled: Requesting and receiving full copies of the following screenshots

and

21/12/15 titled : ACC 45 claim screenshots: How an ACC client can obtain full copies.

It has recently and rightly been pointed out to me that Government Services most probably will not have password access to open the PDF at the website I have referred you to.

Therefore I have attached a copy of the ACC45 Claim Treatment Screenshot PDF (referred to as the PDF acc45screenshot document in my request of 18/12/15) to ensure that there is no unwarranted delay in Government Services responding to my OIA requests on behalf of the Accident Compensation Corporation.

Dear Government Services,

Thank you for your reply.

In response to this request which you have referred to as Request 4, on the last page of your letter under Question IV: Template of Eos screen, you have written and I quote;

Please find an image of a full documents screen from Eos attached, showing all tabs displayed on this screen. For privacy reasons, ACC has blacked out names of identifiable people. The full details though of this screen are provided in accordance with this part of your request.

It is noted that no such image was supplied or attached to your Government Services response received 19/02/16, and therefore I request under urgency that you provide a copy of the Documents screen section to which you refers by no later than the end of business close on 22nd February 2016, to remediate any breach of your obligations under the Official Information Act 1982, that will come into effect at end of business close today.
