How does the University determine the scope of misconduct investigations?

Dear University of Auckland,
There have been several news media articles lately about (belated) investigations into sexual harassment and bullying at the University. In many of them, the University seems to have deemed certain alleged misconduct as being outside the scope of its investigation and/or disciplinary remit.
Once the University becomes aware of alleged bullying or harassment by a member of staff, who decides what the scope of any investigation is?
On what basis is that decision made?
