Just following up with my OIA from October 29, 2025, please see below the request.

​I am writing to request information under the Official Information Act 1982.
​The WorkSafe Board meeting agenda and minutes for December 9, 2024 (Meeting 111) list the following items:

​Item 4.3.1 (Appendix): Serene Heater Review Report ​ The minutes from this meeting state the Board "NOTED and ENDORSED" the recommendations and "endorsed management’s response."
​I formally request copies of the following documents:
​The "Serene Heater Review Report," referenced as Appendix 4.3.1 for the December 9, 2024 Board meeting.
​The "management response" to the review, which was endorsed by the Board at the December 9, 2024 meeting.
​Any reports, updates, or papers on the "review into Serene heaters"
