I'd like to acknowledge your request for “all recorded details of the
meetings and discussions held regarding my SPAD incident, including the
meeting which the Hamilton Union representative was a part of.”

 

Could I please clarify if you are just looking for everything to do with
the incident, i.e. if there were discussions about process and escalation,
safety and management response following the SPAD or are only stuff that
relates to your involvement in the incident i.e documents that mention you
and any actions taken in relation to the SPAD?
