reason, the Ministry of Health has decided to transfer your request to
CCDHB under section 14(b)(ii) of the Act. You can expect a response from

While we are endeavouring to get this OIA response to you within a
reasonable timeframe, Waitematâ DHB’s clinicians, managers and other staff
are concentrating their efforts on measures to manage the current COVID-19
Delta variant outbreak in the region.
It is therefore necessary to extend the time required to provide a
response to your request for information in accordance with section 15A of
the Official Information Act.
This extension is necessary because your request is for a large quantity
of information and a response cannot be collated and reviewed within the
standard timeframe without unreasonably interfering with our operations at
this time.
We will endeavour to provide a response to you by 28 January. This time
extension takes in to account that national shutdown period for the OIA
system which is from COB 24 December 2021 until 17 January 2022.

Thank you for your requests for official information received on 24
November 2021. Your first request was for:
“Surveillance guidelines after removal of colorectal adenomatous polyps
for CCDHB/Wellington Hospital”
This request asks for information which is more closely connected with the
functions of the Capital and Coast District Health Board (CCDHB). For this
reason, the Ministry of Health has decided to transfer your request to
CCDHB under section 14(b)(ii) of the Act. You can expect a response from
CCDHB in due course.
Your second request for official information received on 24 November 2021
was for:
“The percentage of patients that have been declined referrals from
multiple internal and external requests for gastro-intestinal department
-Multiple being more than 3 separate GP/Doctor referrals”
This request asks for information which is more closely connected with the
functions of the District Health Boards (DHBs). For this reason, the
Ministry of Health consulted with the DHBs with the intention to transfer

I understand from the Ombudsman’s Office that in November of last year you
sent an OIA to Capital & Coast District Health Board that was not received
as it was sent to an incorrect email address. I have looked at the FYI
site and have found the OIA request that is outstanding and that you had
referred to the Ombudsman’s office:
Guidelines for following other DHB's, Private Specialist recommendations
for investigation, surveillance, treatment of symptoms/condition once back
under care of CCDHB
Please confirm that this is the request that you are referring to in your
complaint as not being answered and can you please clarify the clinical
field that you are referring to. Once this has been confirmed we will
respond accordingly.
I sincerely apologise for not responding earlier on this matter as my
colleague who was dealing with this has since left and I have been tasked
to follow it up with you.

I am following up with you on an Official Information Act (1982) request
that was originally sent by you to Capital & Coast District Health Board
on 24 November 2021.
My former colleague had informed both yourself and the Ombudsman’s Office
of the issue surrounding the incorrect email address that this OIA was
sent to and the problems that resulted from this. I note that after this
incident we received several OIAs, to the correct email address, from you.
Since my former colleague has left the organisation I have been reviewing
some of the items that he had been working on. It seems to me that he may
have not provided a response to this request. I also note that it is
still on the FYI website as outstanding.
I apologise sincerely for all of this and will ensure it is put right.
The Ombudsman’s Office had made contact earlier in March with CCDHB on
this again, but from the review of this query I noted that it still had
not been responded to. As such I will process this request immediately
and ensure it is responded to in a timely manner.
Thank you for your request for official information regarding Guidelines
for following other DHB’s Private Specialists recommendations, which was
received by Capital & Coast District Health Board. I have receipted this
as received on 24 May 2022.
As required under the OIA, we will respond to your request within 20
working days. You should therefore receive a response on or before 22 June
2022.
If a large amount of information has been requested or if 2DHB needs to
consult in order to make a decision, we may need to extend this date (this
is provided for in section 15A of the Act). We will advise you if such an
