As representative of an ACC claimant, I request the specific information below.
1. Please advise the specific address for service for a Serious Service Failure Request.
Your reply should contain the name, title, department and email address of the individual such a document should be directed to who would subsequently administer such claim.
2. Please supply specific details of what ACC requires to be provided for inclusion in such a process.
Your reply should include each and all information requirements and documentation of such a claim.
3. Please supply the expected time frame a Serious Service Failure Request takes to be assessed and completed when all required documentation has been provided.
Your reply should contain the maximum time ACC considers acceptable to complete this process.
Consultation with Assisted Recovery Team staff has confirmed they have no knowledge of this process, who administers it, and have failed to find out and provide such information despite a specific request to do so. The case manager prior to handing the claim to the Assisted Recovery Team appears to no longer be involved.

Thank you for your reply and the documents provided.
Unfortunately your reply does not provide the answer to my question to solve this issue.
As already stated, the claim file is now in the hands of the Assisted Recovery Team which does not allocate a case manager, does not follow through specific issues with a client via one on one continuity, do not have any knowledge of this process and has, despite a very specific request to do so, failed to find out or supply this required information to be able to request this process be initiated.
So I will ask again, who specifically do I approach to initiate a serious service failure request when there is no apparent case manage to work through?
You may provide me with a contact email address that I can discuss this issue directly with you to achieve a satisfactory result, or you can provide the contact details of a specific person to approach to discuss the Corporation's failure to provide the required specific information via this request.
