Here is PNCC response to your OIA request regarding burial plots.

 

I would like to know how many deceased individuals have been buried
in the wrong plot in Palmerston North since 2000.

One. In fact there has been only one in over 30 years

How long has it taken for the council to be aware of each these
mistakes? 

The error was identified the next day

How was the council made aware of each of these incidents?

The error was identified by Cemetery Operations staff and immediately
escalated to Asset Management staff

What is the protocol for this instance i.e. reburial?

Communicate with affected families and discuss options. Facilitate the
agreed resolution.

What does the council do to ensure these mistakes arent occuring?

Standard procedure has now been changed so that before a plot is allocated
staff triple-check the plot allocation book, the burial register and
computer records.
What are the reasons something like this may happen?

Human error. Technology failure.
