Dear WorkSafe New Zealand,
In the matter of Geoffrey Mark Armstrong, identifying three parties whom acted in the capacity of PCBU and exposed asbestos, contaminating the dwelling and living areas of my family. I wish to gain all material information on file. I wish it to be known that this will be directly compared to the records so far compiled through communications held with both Worksafe staff and the other three parties.
Records may include: formal case notes, images, appointment records, handwritten diaried or other notes, letters and email records between all parties associated with this event. In the case of materials recorded as part of conversations, I will accept WAV, PCM, BWF or MPEG. Should there be any issue with this please identify it to me at your earliest convenience
These records I would prefer to be scanned or otherwise digitally recorded in pdf format and emailed.

This email is to advise you that we will be releasing the information
requested to you. Initially WorkSafe was going to decline your request
under 6(c ) (to ensure maintenance of the law) because a decision had not
been made on whether WorkSafe would investigate. However, we received test
results in late October 2018 and as a result we have decided not to
investigate the matter. This decision was only made in late October 2018
as a result of new information, therefore we are extending your request by
5 days, to 9 November 2018, to enable us to gather all the information you
request.
