2] If Director of Proceedings issuing proceedings against a provider or Doctor through the Health Practitioners Disciplinary Tribunal, and/or the Human Rights Review Tribunal; and/or an informal resolution by way of agreement took place, please provide a copy of the final investigation decision outcome. (names removed)

3] Medical Council of New Zealand processes were on-going in relation and the MCNZ and was asked to keep the HDC informed, Please provide a copy of MCNZ activities final outcome and/or update provided to the HDC. (names removed)
