Dear Wellington City Council,

I am a New Zealand Citizen seeking information on the investigations and reports sought by and provided to WCC in response to the Waterfront drowning of Isaac Levings on 19 April 2023.

Following the inquiry into my son's death in in July 2021 the Coroner made a number recommendations, including that the Council’s response to incidents of people falling from
waterfront should include "a prompt and appropriately robust investigation that is independent of any police or coronial investigation/process”.

I therefore seek the following:

1. A copy of any and all investigation reports sought and/or recieved by WCC and it's various operating entities and Officers in respect of the death of Isaac Levings on 19 April 2023 and/or the events that led up to his death.

2. A copy of any and all draft investigation reports recieved by WCC and it's various operating entities and officers in respect of the death of Isaac Levings on 19 April 2023 and/or the events that led up to his death.

3. A copy of any and all correspondance from and to Wellington City Council staff, Officers, and representatives between any and all independant report writers in respect of the death of Isaac Levings on 19 April 2023 and/or the events that led up to his death.

4. Copies of any and all invoices and or costs incurred by the Wellington City Council in respect of sourcing and receiving any and all independant reports in respect of the death of Isaac Levings on 19 April 2023 and/or the events that led up to his death.

5. A copy of any investigation reports into any and all venues licensed by the Wellington City Council to sell alcohol that were arrended by Isaac Levings on 19 April 2023.

6. A list of any and all prosecutions of venues licensed to sell alcohol by the Wellington City Council and/or their operators in respect of Isaac Levings attendance and/or admission into, on 19 April 2023.
