Dear Ministry of Justice,

 

I have been asked to submit a medical report for Te Whatu Ora on Tuesday
December 15th, 2025 regarding an active Coronial Case.

 

In the interim report [25] states a request for report was made on
February 19th, 2025.

No report was received.

 

1. Who was the initial request sent to please?  Name, email address 2. Was
there a follow up request and if so - Date, name, email address 3. Was any
response received by the Coronial Office from Te Whatu Ora in February?
