Executive 360 Feedback and Related Actions (Last 3 Months)

Dear Accident Compensation Corporation,

Pursuant to the Official Information Act 1982, I request the following information relating to the last three months:

1. Copies of any 360-degree feedback survey results conducted in relation to individual ACC Executive members (including the Chief Executive and Deputy Chief Executives).

2. For each survey completed in this period, I request:
- The full survey output, including:
- Aggregated scores or ratings
- Any benchmarking or comparative summaries (if applicable)
- The verbatim qualitative feedback provided by respondents
- Any reports, summaries, or briefing papers prepared for:
- The individual Executive member
- The Chief Executive
- The ACC Board or any Board committee

3. Copies of any minutes, agendas, or records of ACC Executive Leadership Team or Executive Committee meetings where:
- The results of any Executive 360-degree feedback surveys were discussed, noted, or referenced
-Actions or follow-ups arising from those results were considered or agreed

4. Copies of any remedial, development, or performance improvement plans (whether formal or informal) developed as a result of:
- Low scores, adverse findings, or material concerns identified through Executive 360-degree feedback during this period

I specifically request that anonymous verbatim feedback is included, with only:
- Names, email addresses, or other direct personal identifiers of individual respondents removed where necessary to protect privacy.

I acknowledge ACC may consider withholding information under sections 9(2)(a), 9(2)(ba), or 9(2)(f)(iv) of the Official Information Act. However, I note that:
- The information relates to senior public-sector executives exercising significant public authority
- The request seeks anonymised feedback, not the identification of respondents
- There is a strong public interest in transparency and accountability regarding executive leadership, organisational culture, and the effective use of public funds at ACC

If ACC considers that any information cannot be released in full, please:
- Identify the specific statutory grounds for withholding each item
- Consider whether partial release or redaction would address any concerns
- Provide the public-interest balancing analysis relied upon where section 9 is applied.
