Your Official Information Act request, reference: GOV-045118 Thank you for your request of 13 December 2025, asking for the following information under the Official
Information Act 1982 (the Act):
I request the following information relating to the last three months:
1. Copies of any 360-degree feedback survey results conducted in relation to individual ACC
Executive members (including the Chief Executive and Deputy Chief Executives).
2. For each survey completed in this period, I request:
•
The full survey output, including:
o
Aggregated scores or ratings
o
Any benchmarking or comparative summaries (if applicable)
o
The verbatim qualitative feedback provided by respondents
•
Any reports, summaries, or briefing papers prepared for:
o
The individual Executive member
o
The Chief Executive
o
The ACC Board or any Board committee
3. Copies of any minutes, agendas, or records of ACC Executive Leadership Team or Executive
Committee meetings where:
•
The results of any Executive 360-degree feedback surveys were discussed, noted, or
referenced
•
Actions or follow-ups arising from those results were considered or agreed
4. Copies of any remedial, development, or performance improvement plans (whether formal
or informal) developed as a result of:
•
Low scores, adverse findings, or material concerns identified through Executive 360-
degree feedback during this period
I specifically request that anonymous verbatim feedback is included, with only:
•
Names, email addresses, or other direct personal identifiers of individual respondents
removed where necessary to protect privacy.
If ACC considers that any information cannot be released in full, please:
•
Identify the specific statutory grounds for withholding each item
•
Consider whether partial release or redaction would address any concerns
•
Provide the public-interest balancing analysis relied upon where section 9 is applied.

Questions one and two The information you have requested is personal information about ACC staff members. We are
therefore withholding this information to protect the privacy of the individuals involved. This decision
has been made under section 9(2)(a) of the Act. In doing so, we have considered the public interest in
making the information available and determined that it does not outweigh the need to protect the
individuals’ privacy.
Questions three and four We are refusing your request, as the information does not exist. This decision is made under section
18(e) of the Act.
