Dear Accident Compensation Corporation,

Under the Official Information Act 1982, I request the following information:

1. Any internal policy documents, guidelines, audit summaries, or quality assurance procedures that reference or govern the override, substitution, or non-acceptance of an ACC18 medical certificate, particularly in relation to:

- Weekly compensation eligibility;
- Permanent Injury Compensation (PIC) decisions;
- Disputes or reassessments.

2. A list of any Employment Relations Authority (ERA) or Dispute Resolution Services (DRS) cases since 2015 in which the ACC18 form or override of provider certification was raised, contested, or referenced.

3. Any summary data or internal dashboards showing rates of:

- ACC18 overrides;
- ACC18-to-PIC conversion denials;
- Related internal audits or escalations.

If any of the above is not held, please confirm this under section 18(e) and describe what reasonable steps were taken to determine this.
