Dear Health and Disability Commissioner,
In line with the Official Information Act 1982, I am requesting the following details-
1. Copies of documents describing the selection criteria or framework used to determine the 2025 proposed role changes or disestablishments.
2. Copies of any internal or external advice received (e.g. legal, HR, or strategic) in relation to the 2025 restructure.
3. Any financial modelling, cost-benefit analysis, or savings forecasts prepared to support the proposed restructure in 2025.
4. Any internal audit reports, reviews, or risk assessments related to financial governance or restructure planning over the last 3 years.
5. Documentation relating to previous restructures or financial reviews in the past five years, particularly where issues of overspending, audit recommendations, or financial mismanagement were raised.
6. Documents relating to any reviews or evaluations undertaken to assess the effectiveness or efficiency of the leadership structure.
7. Vacancy rates for frontline positions, including investigators and complaints assessors over the past 3 years.
8. A breakdown of total annual expenditure over the past 3–5 years, segmented by:
A. Rights promotion (e.g. education campaigns, outreach, public engagement, communications)
B. Complaints resolution or investigations.
