Thank you for your response. May I first note that WorkSafe New Zealand
has no record of any communication relating to this request dated 21 May.
Our records indicate that the request was received as below and we are
working from that date:

WorkSafe intends to respond to the Mayor’s request in tranches as I will
explain below.
1. Fatalities.
The data for fatalities for the requested period will be released to you
before the end of this week.
2. Serious injury data - 1
This will be released in two tranches. The first tranche will be material
from 2011 – 2014. This has previously been prepared for another purpose
and has been reviewed for accuracy. This will be released to you early in
the w/c 6 July.
3. Serious injury data - 2
This data does not exist in the form in which you request it. To provide
this information will require the interrogation of our databases and the
manual review of each file resulting from that process. This manual
review is required because prior to 2011, data was not input in a manner
which immediately identifies it as you request. This process will not be
able to be completed within the statutory timeframes laid out in the OIA.
We are therefore notifying you of our intention to extend the due date by
10 working days on this element of your request in reliance on Section 15A
of the Official Information Act. The request was originally received 19
June which would have meant it was due to you on 17 July. The extension
puts the due date to you at Friday 31 July 2015.
