Hello again Ian
The workgroup responding to your request have asked if you can kindly clarify your request "please supply sudden death documents known as POL47, between February 2020 and December 2023".
Do you want copies of the blank POL47 form used from 2020-2023 to look at changes to the form content - or do you want copies of all completed copies of POL47 forms used from 2020-2023?
If you are seeking the later, the workgroup has asked if you wish to refine/narrow your request, given there are over 5,000 callouts to sudden deaths per annum, so as it stands your request would be a refusal under 18(f) substantial collation. Is there a particular location you are interested in or specific age group etc?
If you can please reply by 29 February 2024, otherwise Police will progress a response with the interpretation being for completed copies of the POL47 forms.
Ngā mihi, Michelle

For this reason, Police has decided to partially transfer your request to the Ministry of Justice under section 14(b)(ii) of the Act. You can expect a response from the Ministry of Justice in due course.
Portion of request for transfer:
all completed POL47 forms from February 2020 to December 2023 for the Hamilton area
Under section 28(3) of the Act you have the right to ask the Ombudsman to review any decisions made under this request. The Ombudsman may be contacted by email at: [email address] or by calling 0800 802 602.

I’ve now heard back from the team at this end regarding information that
might be available regarding your request about POL47 forms.
I understand it would be possible to let you know the number of closed
cases notified to the Coroner between 1 February 2020 and 31 December 2023
that had a POL47 attached.
This could be broken down further as follows:
o Region – Waikato HNZ
o Age
o Gender
o Cause of death
Can you please let me know if these figures would assist you.
Thank you for your help with this.
Ngâ mihi
