This email is to acknowledge Hawke's Bay DHB has received your official information request (below) dated 25 May 2021 and assigned the reference OIA2021059.

Thank you for your Official Information Act request, dated 25 May 2021
requesting the following information:
 
“We seek a copy of the most recent performance management report you have
received from HealthCare NZ Limited.

Limited receives from Hawke's DHB for missed care”.
 
Hawke’s Bay District Health Board (HBDHB) believes that the information
requested is more appropriately addressed by The Ministry of Health as the
request is more closely connected with the Ministry of Health’s function. 
Therefore HBDHB is transferring your request to them under Section 14 of
the Official Information Act 1982.
 
You have the right, under Section 28 of the Act, to ask the Ombudsman to
review the decision to transfer your request.  Information about making a
complaint is on the website of the Office of the Ombudsman
([1]www.ombudsman.parliament.nz).  The Office can also be contacted on
freephone 0800 802 602.
