Number of un-announced inspections

Dear Office of the Ombudsman,
I am requesting to know how many un announced inspections have been conducted / performed in:
1) Hospitals (including situations where people are confined in the name of Medicine under the Mental Health Act)
2) Prisons.
Per year.
Lets go from this year (so far) back 2020, 2019, 2018, 2017, 2016.
I know of only 1 un announced inspection that has been conducted.
I am wondering whether this (1 un announced inspection) indicates an improvement on the previous situation (since none were performed un announced previously) or whether there always have been, on occasion, un announced inspections.

I understand that the Office of the Ombudsman is not subject to the Official Information Act.
But that is not to excuse it refusing to perform it's statutory function with reasonable care and skill.

Dear Kelly,
You are correct, the Ombudsman is not subject to the Official Information
Act, however he acts in a manner which is consistent with, and in the
spirit of the Act.
The internationally accepted standard is that one third of inspections are
unannounced. Each year, over the past five financial years, this standard
was exceeded by a significant percentage. On average 76% of the
inspections conducted since the 2015/16 financial year were unannounced.
The table below breaks down the number of visits and the percentage that
were unannounced each year:

Each year in the Ombudsman’s annual report he provides information about
