Dear Capital and Coast District Health Board,

I would be grateful if you could provide me with the following information:

1. The total number of hemipelvectomies performed by the CCDHB over the last 5 years and (i) the total cost of those surgeries and (ii) the total rehabilitation costs to date associated with those surgeries;

2. The total number of pelvic restoration surgeries performed by the CCDHB over the last 5 years (for example due to osteosarcoma, motor vehicle accident trauma and the like) where those surgeries used custom implants and (i) the total cost of those surgeries and (ii) the total cost of the custom implants used in those surgeries; and

3. The total number of (i) spinal fusions and (ii) spinal fusions using anterior lumbar inter-body fusion (ALIF) devices for each of the last 5 years by the CCDHB and (iii) the total cost of those surgeries for each year and (iv) the total cost of the ALIF devices for each year.
