Dear Department of Conservation,
I request the Department of Conservation to provide the following information in relation to Conditions outlined in the “Authority to Have in Possession Protected Wildlife” the Department has granted to the Otorohanga Zoological Society Incorporated (OZS) (current Authority signed and dated July 2012: file reference NHS-02-01-02-03; Authority number: WK-21005-CAP; DOCDM-773725; and the previous version thereof)
1. For calendar years 2012 and 2013, a summary of all deaths of Protected Wildlife reported by OZS to the Maniapoto Area Manager within 72 hours of death or discovery (first part of Condition 15). Please specify the date of death and species of wildlife. Please also specify the date of receipt of these reports.
2. For calendar years 2012 and 2013, a summary of all deaths of Protected Wildlife reported by OZS to the Maniapoto Area Manager within one month of death or discovery (second part of Condition 15). Please specify the date of death, species of wildlife and cause of death reported. Please also specify the date of receipt of these reports.
3. Copies of annual reports submitted by OZS over the last five years of recorded numbers and species of Protected Wildlife held, including births, deaths and exchanges between captive locations (Condition 19). Please specify the date of receipt of these reports in relation to the expected submission date of 31st July each year.

I understand that the Department of Conservation (Te Kuiti office) has received, since the time of my original OIA request of 19 December 2013 further information, pertinent to my request, from the Otorohanga Zoological Society.
I therefore now request that the Department of Conservation provide under the OIA this follow up information to my original request. In particular I request the annual report due on 31st July 2013.

The information requested was incomplete. This is due to the fact that the Otorohanga Zoological Society appears to have failed to meet the requirements of the permit to hold protected species- and has not yet submitted the annual report for 2013. It is not yet known whether DOC will follow this up. The conditions of the permit require an annual report to be filed by 31 July of each year.
