Clarification on MBIE's recruitment process

This FYI request is a result of my communication with MBIE's Talent Acquisition manager in the HR unit in regards to vacancies for client Services Advisor roles.
On the 2 occasions of vacancies (August and October) for the CSA roles I requested reasons why my applications were declined and on both occasions I received almost identical responses from HR - both of which did not initially answer the question.
I do find it strange that the circumstances of two different rounds of vacancies produced the exact same response - even the number of applicants were almost the same 100 and 98.
I believe this indicates a lack of robustness, fairness in the current recruitment process and leaves room for discrimination hence my FYI request below.
1. What is the process when declined applicants request a review of decision?
1b.What is the timeframe & how is this response provided to the applicant?
1c. When was the last time this review process was 'updated'?
2. What is the process when applicants request the reasons for their application being declined?
2b. What is the timeframe & how are these reasons provided?
2c. How is this response provided to the applicant?
3. How long have these processes existed for within your department?
4. There were 2 round of vacancies (in August & October) advertising for the position of Client Service Advisor and I had queried the recruitment process after the first round of vacancies in August.
I had asked why there were additional questions installed in the second round of advertisements for the CSA role and the response I received on 16/10/17 failed to answer the question, instead, I was advised that they have been introduced in the second round of vacancies - a point I was already aware of and hence why I asked the question in the first place.
Why were there additional questions in second round of vacancies (October) for the CSA role(MBIE/1298915)?
5. Were there two different applicants hired for both August(MBIE/1289728) and October(MBIE/1298915) vacancies or was the August vacancy merely re-advertised in October?
