Request for a copy of the Clinical Services Contract, and all policy documentation for the Single Discipline Assessment SDA 30 that was in force as at 05 August 2014

David Lawson made this Official Information request to Accident Compensation Corporation

Response to this request is long overdue. By law Accident Compensation Corporation should have responded by now (details and exceptions). The requester can complain to the Ombudsman.

From: David Lawson

Dear Accident Compensation Corporation,

I respectfully request the provision of copies of all policy documentation that was associated with ACC's service contract for service code SDA30, service description: Orthopaedic Assessment that were associated with this service between 1 June 2014 and 23 December 2014.

Please ensure that a copy of the service contract is included that was applicable through the period requested, and also confirms the number of hours that the SDA30 was allotted for the completion.

I respectfully request that this information since it is readily held by the Corporation, is made available within 10 working days, being by the end of business close on 4th August 2017, with the reason being that it is required for pending review and appeal purposes, together with being required for the completion of an application for review of ACC's decision making and or lack there of providing me with review rights.

I thank you for your time and assistance and advise that I under s16(2) of the Official Information Act, my preferred way of receiving a response is by email to the address from which ACC received the original request, not by post.

Yours faithfully,

David Lawson

Dear Mr Lawson

Thank you for your email dated 22 July 2017, copied below, requesting
information under the Official Information Act 1982 (the Act).

ACC is working on your request and will be in touch with you as soon as
possible, and certainly by 18 August.

We have noted your request for receipt of the information by 4 August. If
we are able to accommodate that time frame, we will.

While the Act allows a requester to ask for a request to be processed
urgently (where they specify the reason for this), agencies are not bound
by such a request. Nor does the Act provide for the requestor to
unilaterally impose due dates in relation to a request for urgency.
Whether a response can be provided under urgency will depend on various
factors, including the complexity of the request, the time required to
form a response, and the workload of the agency.

If you have any questions in the meantime, we will be happy to work with
you to resolve these. We can be contacted via email at:
[1][email address].

Yours sincerely
