Dear Accident Compensation Corporation,
A former employee of ACC I have spoken with recalls seeing an ACC document regarding an evaluation of acupuncture for the rehabilitation of injury, which contained purchasing advice for acupuncture treatments. I would like a copy of this document, but I have been unable to find it on the ACC website.
The document would have been published prior to 2011, and may have since been superceded by a more recent document. I believe it is likely to be a Considered Judgement Form.
I'm not sure if this is likely to be enough information to find the document I'm looking for, or what other information I might be able to supply that would be useful. If this information is insufficient to locate it, I'd be happy to discuss what other information you might find useful in identifying it or ways in which my request might be refined.

A response to this request was due no later than the 27th of February. It is now the 28th of February, and I have received no response. It would be great if you could send me the document I've asked for as soon as possible. Given the lateness of the response, and the lack of communication, I intend to lodge a complaint with the Ombudsman.

My apologies for any confusion my email from this morning may have caused. I have been subsequently informed that there was an issue with the processing of incoming mail on the FYI website that I have used to make this request. The result of this was that your message from yesterday was not processed and made known to me until today. This was not your fault in any way.
As such, I realise it would not be appropriate for me to complain to the Ombudsman regarding the lateness of your response, and no longer intend to do so.
