The request was successful.
From: Antony Andrews
Dear Ministry of Health,
I thank you for providing me with the data that I requested on 12 August 2016, under H20160 3264.
Upon working with the above data, it has come to my attention that I would need the same quarterly data for periods prior to Sep 2013.
My new request under official information act is that; if you could provide quarterly data on the following three SCHEDULES for periods 30 June 2000 to 30 June 2013:
SCHEDULE 3: DHB Provider Arm Financial Performance
SCHEDULE 4: Average Year to Date Consolidated Accrued Full Time Equivalents (FTEs)
SCHEDULE 7: Capital Expenditure
I do understand that the time frame for this data goes back to 2000, if there are any operational difficulty in getting old data, please let me know.
When providing the data if you could provide in excel format that will be great for handling purposes.
I thank you for your assistance so far with my earlier requests.
Yours faithfully,
Antony Andrews

Ministry of Health
Hi Antony
In this OIA you have requested the information for the period 30 June 2000
to 30 June 2013 (quarterly)
As DHBs were set up in 2001 we have no data prior to this.
The schedules evolved over time as follows:
Schedule 4 (average FTEs) was first used in December 2004.
Schedule 3 (Provider Arm financial performance) was first used in
July 2005.
Schedule 7 (capital expenditure) was first used in November 2006.
The schedules themselves evolved over this time as well so they are not in
exactly the same format as they are at present.
As you can see we cannot answer your OIA for the complete time period you
have asked for. Prior to the dates listed above, the information is not
held by the Ministry Of Health.
Please let me know over what time period you would like the information.

From: Antony Andrews
Dear David,
Thank you very much for your reply and clarifying the availability of data.
So if you can provide the quarterly data from November 2006 on Schedule 4 (average FTEs), Schedule 3 (Provider Arm financial performance) and Schedule 7 (capital expenditure) that will be great.
I also acknowledge the fact that format might be different.
While providing the data if you could please provide in excel that will be great.
Once again that you very much.
Yours faithfully,
Antony Andrews
