How many times has IRD requested information from NZ foreign trusts in the past year?

Dear Inland Revenue Department,

According to the Prime Minister, tax-free New Zealand foreign trusts must retain detailed information on their activities and provide it on request to the IRD ("NZ government not digging heels in over foreign trusts - PM John Key". Stuff, 5 April 2016). I would like to request the following information under the OIA:

* How many times in the last year did IRD request information from an NZ foreign trust?
* How many times was such a request successful
* On how many occasions was information requested from an NZ foreign trust disclosed to a foreign government? If the answer is more than zero, I would like a breakdown by country.
* If any request for information was not successful, what action was taken, if any?

I would prefer to receive an electronic response. Queries about
this request will be automatically forwarded to me by the
fyi.org.nz website.

With regards to s12 of the OIA, I am an NZ citizen and in NZ.
