Subject: Official Information request - Risk assessment at UoA for
Covid-19 vaccination policy

 
Dear University of Auckland,
At the start of the 2021 school year, the university required covid-19
vaccination to attend campus which is published here:

"Thursday 3 February 2022

Before coming on to any University of Auckland campus
Students, staff, your My Vaccine Pass must be uploaded into the
University's Vaccination Declaration Portal.
This also applies to contractors and volunteers who have a University
username.
Visitors must bring a digital or hard copy version of their My Vaccine
Pass, and be ready to present it to University officials upon request at
all times while on campus.
This also applies to contractors and volunteers who do not have a
University username."

Under the Official Information Act, I request a copy of the Health &
Safety risk assessment that the University would have had to undertook
that outlines this outcome to require vaccination.  If the health and
safety risk assessment does not include the following, would you please
share the assessment details of

a) all the locations of the hazard, (the  Covid-19 disease) that was
reported at UoA workplaces,
b) the data of the hazard's occurrences,  eg hazard reported at med school
lab on xx date
c) the various controls that were assessed and their effectiveness
d) the hierarchy of controls for the hazard and which control had the
lesser of risks
