Infant Death P47 form/report

Dear Ministry of Justice,

I am looking for the form that is filled out by police at the scene of an infant's death. It is referred to as the P47 or P47a report, or P47 or P47a form in all the literature. It collects information about the death scene in a checklist format and is used alongside the post-mortem autopsy report for the coroner to determine the cause of death.

I look forward to receiving this information.

I am after a blank form, I am interested in what the questions on the form are, not in a particular case itself. I cannot find where to request this from the Police and was directed here.

I have still not heard from the New Zealand Police in regards to being sent a copy of the P47 form used at the scene of an infant death. By law they should have responded by the 24th of December.

Could you please get in touch with them again and ask them to respond to my request as soon as possible.

A response by Police is due to you by 15 February 2016 (this takes into account the summer holiday -27 Dec to 15 Jan, as well as Waitangi day).
