Pursuant to the Official Information Act 1982, I request the following information:
1 - The eligibility criteria for individuals to access publicly funded support services through SupportNet in the Bay of Plenty region.
2 - Any operational guidelines, referral pathways, or assessment tools used to determine access to SupportNet services.
3 - Information on whether there are priority groups or conditions (e.g., age, disability, mental health status) that influence access or waiting times for SupportNet support.
4 - If available, the most recent version of any policy documents or standard operating procedures that govern how access to SupportNet is managed.
5 - Details on the interface between SupportNet and other services, such as Te Whatu Ora, NASC (Needs Assessment and Service Coordination), or NGOs providing regional mental health, disability, or community support.

Support Net has two pathways for individuals to access to publicly funded
support services. These are for:
* people (of all ages) with long-term chronic health needs, or
* those aged over the age of 65 years and have long term disability or
age-related health needs.
In order, for us to respond, could you please specify which pathway you
are seeking the information for (ie, under 65 years or over 65 years old).
It would be appreciated if you could provide a response by 5.00pm on 1
December 2025 so we can progress your request. N
