Dear Health Quality and Safety Commission,
I understand Fale Andrew Lesā was appointed to the Child and Youth Mortality Review Committee in August 2016 and that he completed two terms on the committee as a youth advisor and consumer representative and his terms concluded in June 2023. I further understand that between 1 June 2023 to June 2024 Mr Lesā agreed to become a 'subject matter expert' for child and youth mortality.
There is an established and considerable public interest in Mr Lesā's appointment history owing to media coverage relating to improper claims made by Mr Lesā as to his experience, expertise and qualifications.
I request under the Official Information Act the following information:
- An explanation as to why Mr Lesā was deemed suitable to be appointed to this position.
- All documents pertaining to Mr Lesā's appointment including a) any CVs, application forms, or statements or information supplied by Mr Lesā in the course of the appointment process b) any advice or reports produced by the Committee in the process of appointing Mr Lesā.
- A record of the total amount of funds paid by the Committee to Mr Lesā during his appointment.
- How many meetings Mr Lesā attended, and how many meetings he did not attend during his appointment.
- Any information/complaints/concerns pertaining to Mr Lesā raised with the Committee before, during, or after Mr Lesā's appointment.

Hi there,
I found this online.
I was a referee when Fale initially applied to the committee. I encouraged them not to hire him - not because I suspected deception but because in my experience (having worked with him in a governance group for some time) he was disconnected from the population he claimed to represent and did not have expertise, despite looking good on paper. I was very clear about my concerns and they acknowledged them.
I do not wish to put my name here right now but will keep an eye on this page to see if there are updates or another way of speaking more privately.
