Dear Ministry of Health,

For the last 5 years, please provide monthly GP & nurse visits by PHO.
Annual data was provided in the data request "data on patients enrolled at
all GP practices, number of visits and booking delays".

If there are further details on visits, i.e. gender, ethnic group (Maori,
Pacific, etc.), or age group, please provide this as well.
